New & Established Patient Info

Step 1 – Schedule an initial new patient evaluation & complete a new patient form: If you are ready to schedule your patient appointment, please call the office number at 757-299-0598 to schedule a 60-minute 1-on-1 assessment session to understand your symptoms and get a professional diagnosis. This first session costs $255 in-person at our clinic or virtually/telehealth (some in person might be needed occasionally depending on the treatment plan). If you get voicemail, please leave a message and allow 24 hours (1 business day) for a call back. Alternatively, you can request an appointment via email at: info@hamptonroadspsychiatry.com. Prior to your appointment please complete the New Patient Form below: PATIENT FORM LINK. Return New Patient Forms prior to appointment via email at: info@hamptonroadspsychiatry.com.

Step 2 – Get a treatment plan at your initial evaluation: Based on your evaluation and diagnosis, we’ll prescribe a treatment plan that can include recommended medications and/or specific individual therapy options.

Step 3 – Check in with your provider and schedule a follow-up appointment: Sometimes it takes a while to find the right treatment options for you. We’ll meet at least every one month to every three months when stable to make sure your treatment plan and/or medication is working and to support you as you adjust. Medication management appointments are at least every four weeks until the appropriate medication and effective dosage is established. These visits are also to ensure that the medications are tolerated without side effects. Medication management appointments for patients who have a stable medication regimen are approximately every 3 months/quarterly. Individual therapy only patients (not on medications) are typically seen weekly or bi-weekly, but the frequency of these sessions is tailored to each individual patient’s needs.
