Payment & Insurance Information & policies

• At this time, we are out of network and only see self-pay (out of pocket) patients. Patients are responsible for the full cost of the appointment due prior to the visit. Patients can be provided a superbill upon request if they wish to submit it to their health insurance plans for possible reimbursement. Please be prepared to provide your name, valid home address, and contact information (phone & email) to secure your patient appointment date. On the day of service, please be prepared to provide your credit card information (Visa, Mastercard, American Express, Discover) and to be charged for the full cost of the visit. If payments are not valid at that time, the appointment must be paid for with another method prior to the appointment or it will be canceled. Fees for self-pay are as follows:
  1. New Patient Evaluation: $258
  2. Established Patient 30-minute Session: $128
  3. Established Patient Therapy Only 60-minute Sessions: $200
